Peralta lined a single to center off loser Jim Johnson (4-6), the Orioles sixth pitcher, to bring in Luis Valbuena.
Peralta had three hits and two RBI as the Indians won their second consecutive game against the Orioles. Cleveland, which had dropped 11 straight before the O's came to town, can sweep the three-game series Sunday.
Shin-Soo Choo led off the fourth by putting one out to right, then Andy Marte hammered a two-run homer to left.
Valbuena finished with a single and a double, scored two runs and had two RBI, while Asdrubal Cabrera drove in one.
For the Orioles, Robert Andino led off the fifth with a homer to left and Luke Scott blasted a three-run shot to right in the same inning.
Michael Aubrey also homered, while Melvin Mora and Matt Wieters each drove in a run.
Kerry Wood (3-3) got the win, despite giving up sac flies to Nick Markakis and Mora, allowing the Orioles to tie the game 8-8 in the ninth. Wood, who pitched the ninth, gave up two runs on three hits and walked one.
