Country singer Mindy McCready reportedly has entered an extended-care facility after visiting an emergency room in a Nashville hospital last week. The 32-year-old troubled singer checked into the emergency room in Tennessee on July 16 after suffering a nervous breakdown, and is now in rehab.

McCready’s rep though confirmed to People.com that the country music singer entered rehab, but did not reveal the reasons for this.

"Mindy McCready did receive emergency care at Vanderbilt University Medical Center," said the rep. "Other issues made it prudent for her to seek longer-term and broader-based treatment.”

North Fort Myers native McCready, who back in April admitted to having a long-term relationship with New York Yankees star pitcher Roger Clemens, suffered the nervous breakdown due to a series of emotional events she has gone through recently.

She has now entered the rehab for undisclosed reasons, the rep said. "To her credit, she has entered into an extended-care facility where she will receive the care needed," said her rep. "And for the moment we ask that everyone respect her privacy."

Although the reason for McCready’s hospital visit was not confirmed, but TMZ.com reported Wednesday that McCready, who has a history of substance-abuse issues, had entered an Arizona rehab facility after mixing vodka and sleeping pill Ambien in an alleged suicide attempt.

The past few years have seen McCready in the news for all the wrong reasons, including the personal and legal matters.

In 2004, McCready was arrested n Tennessee for using a fake prescription to buy the painkiller OxyContin. Although she initially denied the charge, she eventually pleaded guilty the same year in November. She was then fined $4,000 and sentenced to three years probation. She was also ordered to perform 200 hours of community service.

In 2005, McCready was stopped by Nashville police for speeding and then arrested and charged with driving under the influence (DUI) and driving with a suspended license. The country music artist was later released on $3500 bail.

In July 2005, McCready was found unconscious in a hotel lobby in Indian Rocks Beach, Florida. She was hospitalized due to a drug overdose after consuming a large amount of undisclosed drugs with alcohol.

In 2007, she was sent behind bars for violating probation on the 2004 drug arrest.

In April, 2008, she admitted she had a long-term relationship with Clemens, who is at the center of a steroid controversy. The two met in a Florida karaoke bar when McCready was a 15-year-old aspiring singer and Clemens was a 28-year-old ace for the Red Sox and a married father of two. Clemens, the seven-time Cy Young Award winner, is under FBI investigation for perjury after denying, under oath, accusations by his former personal trainer Brian McNamee that he used performance-enhancing drugs. The FBI probe also involved questioning the country singer.

McCready is currently in a custody feud with her ex Billy McKnight over their 2-year-old son, Zander Ryan, who is in the care of her mother, Terri, since last fall.
