Jayson Werth drove in Howard with the winning run, capping a Phillies comeback after the Rockies scored three in the eighth to take a 4-1 lead.
The defending World Series-champion Phillies won the best-of-five NL Division Series in four games, setting up a rematch of last season's National League Championship Series between the Phillies and LA Dodgers. Game 1 is scheduled for Thursday in Los Angeles.
The LA Angels and New York Yankees will meet in the American League Championship Series beginning Friday.
After Chase Utley drew a two-out walk in the top of the ninth off Rockies closer Huston Street (0-2), Howard drove a 2-1 fastball over the head of Carlos Gonzalez in right to score Shane Victorino and Utley to tie the game at 4-4. Street was charged with all three runs on three hits with one walk.
Werth then drove in Howard to give the Phillies a 5-4 lead.
Colorado put two runners on base against Scott Eyre in the ninth before Brad Lidge came on to strike out Troy Tulowitzki for the final out. Ryan Madson got the win.
Colorado had taken the lead with a three-run outburst in the bottom of the eighth when pinch-hitter Jason Giambi hit a score-tying single and Yorvit Torrealba followed with a two-run double for a 4-2 Rockies lead.
