Oscar Salazar hit a three-run, pinch-hit homer for the Orioles and Felix Pie added a pair of RBI.
Kevin Youklis and Dustin Pedroia had three RBI apiece for the Red Sox in John Smoltz's second start of the season. Smoltz gave up one run on three hits in a start limited to four innings by a rain delay of 1 hour, 11 minutes.
Boston came out strong with a pair of runs in both the first and second innings.
The Red Sox added five runs in the fourth inning to take a 9-1 lead and were up 10-1 after seven, Baltimore roared back with five runs, including the three-run shot by Salazar.
In the following inning, the Orioles got to Boston closer Jonathan Papelbon with a two-run double by Nick Markasis that handed Papelbon his second blown save this season.
Takashi Saito (2-1) also pitched a portion of the eighth and took the loss.
Mark Hendrickson (3-4) got the win with three innings in relief. George Sherrill picked up his 17th save.
